We have included some additional questions related to:

Risk Assessments Many activities and events need one, this protects people and your ministry.
Who else is involved? You might invite the help of others such as independent ministries, or even a food vendor. It is best practice to also ask them for a copy of their current Public Liability Certificate of Cover.
Likewise, qualified professionals might be actively contributing to your event or activity. Where they are providing services in their professional capacity, RMS may ask for evidence of appropriate insurance cover such as Professional Indemnity Certificate of Cover. Again, this is best practice to protect your ministry.

It is no drama if you can't submit the above documents when you complete this form, but we kindly ask you to please be understanding if one of our team asks you to provide all or any of these documents while they process your request.
